Is IV iron safe to administer in acute infections?

Answer: Yes. The longstanding concern that IV iron might exacerbate acute infection is not borne out by recent data. In a large retrospective cohort (TriNetX Research Network, 2000–June 2025) of adults with iron-deficiency anemia and one of five infections (MRSA bacteremia, pneumonia, UTI, colitis, or cellulitis), IV iron was associated with better outcomes after 1:1 propensity matching within each infection cohort:

  • Higher survival at both 14 and 90 days across all five infection types (P < .001 for each) — e.g. pneumonia 95.7% vs 91.5% at 14 days and 84.7% vs 78.1% at 90 days.
  • Greater hemoglobin recovery at 60–90 days in every subgroup (all P < .001) — e.g. colitis +1.5 vs +0.7 g/dL.

Bottom line: IV iron during acute infection did not exacerbate infection and was associated with improved survival and anemia recovery. Being retrospective, it establishes association rather than causation — prospective studies are still needed to confirm.
